Below you will find forms that students are required to fill out annually as well as information and links for Insurance if needed.

Student Insurance Brochure
**This insurance is only needed if you do not currently have personal insurance. Below is the process you will need to follow to apply.
Scroll down the page to School District and select “Maine School Management Association (ME)”
a. A new “Program Coverage” drop down will pop up. Select “School Time”.
On the new page titled “School Time”, the $11 option is the minimum required. If you wish to purchase further coverage, that is an option also. Select the option you wish to purchase.
Complete the Student Name/Address info.
Complete Parent/Guardian info.
Read Voluntary Consent page, click that you have read and agree to terms. Add email and date.
Read Fraud Warning page, click that you have read notice.
Enter info in “Make a Payment”.
Print off a copy of receipt or proof of purchase and turn that into your instructor OR Karri in the Foster CTE office.
